Referral Process/Client Intake

The Center primarily receives referrals from the Adult Probation Department and Social Service Department of the Circuit Court of Cook County and the Circuit Court of DuPage County and its Adult Probation Department, as well as occasional referrals from courts in other jurisdictions including some from other states. 

CADP also receives referrals from child welfare (DCFS contractors), other social service agencies, private practitioners, attorneys, and from individuals who believe they have been abusive to an intimate partner and want to change. 
 

First Call: Call our Tri-Taylor office at (312) 265-0206 (813 S. Western Avenue, Chicago, Illinois) for services in any of our Chicago locations. Call our Wheaton office (331) 716-7995 office for our Wheaton location. At those numbers you can make an appointment for a confidential assessment. At this time and at least through May 2020 all services are being provided remotely and a staff person will ask you for information so that we can set up an online or telephone assessment appointment. A sign-in sheet and other documents will be sent to you through a secure system that permits you to fill them out on line and send them back to us. The appointment will take approximately two hours and our assessment worker will work with you to determine if you are appropriate for our program.

  1. If your intake worker determines that these groups might be helpful for you to change some patterns in your life that have become destructive, you will be asked to attend an an online orientation group that will introduce you to the group process. Documents and a link to log in to the group session will be sent to you. The staff member who contacts you will instruct you about how to log in and how to pay for your sessions.

  2. Following orientation, you will begin weekly 2-hour groups. A sign-in sheet and a link to log in will be sent to you each week.

  3. The group program continues for a minimum of 24 weeks, but may be extended if needed to meet completion criteria.

  4. Certificates are provided for successful completion of the program.
